Directions & parking

Our College Avenue campus has free on-site parking, including accessible spaces near the main entrance.

If you're visiting for a tour or intake meeting, please call ahead so we know to expect you. Our front-desk team will direct you to the appropriate office or program area.

Wheelchair accessible. All program areas are accessible. If you need additional accommodations for your visit, let us know in advance.

Grievances

If something isn't right, we want to hear about it.

Caldwell Opportunities is committed to providing services that respect every participant's rights and dignity. If you, your family member, or your loved one has a concern or complaint about our services, we want to know — and we have a clear process to address it.

How to file a grievance

  1. Speak to staff directly. Most concerns are resolved quickly when raised with the program coordinator or staff member involved.
  2. Escalate to the Program Director. If the concern is not resolved, request a meeting with our Program Director.
  3. File a formal grievance. Submit in writing to the Executive Director. We'll acknowledge within 2 business days and respond fully within 14 days.
  4. External review. If you're not satisfied with our response, you have the right to contact your LME-MCO, NC DHHS, or CARF for independent review.

Confidentiality: Grievances are handled confidentially. Filing a grievance will never affect the quality of services your loved one receives.
